You can decide for yourself where you want to put the slides/hammer-ons/pull-offs in the intro, it really doesn't matter that much. Chorus: For the melody guitar, just play the intro over the vocals, you can hear where it goes. As for the chords...

If we can make this... Then we can break it...The above chords part gets played two times through obviously. Just follow the lead guitar with it and you'll be good to go. You don't have to mute the low E string if you don't want. In that case you could use the standard power chords, Drop-D power chords, or you could even tune the low E-string 1/8 step above C# and have a nice low root bass always there. I'd recommend the third out of those three, then for the rest of the chords in the song you could just leave the low E string open and explore some different chord voicings.
Obviusly you'd just play the same chords for the chorus as before, just two frets up then before. Since a synthesizer or something like that backs the verses and not guitar, you really can't do much guitar over them anyway except for once you hear them come in with the guitars themselves. Here's the best I've got so far... Verse 1:
